We're at an inflection point most of the industry hasn't priced in yet. AI has made building software fast and cheap - but in health care, speed only matters if it holds up under audit, works at population scale, and earns a clinical team's trust before it earns their adoption.
That's the thinking behind V2R™ - Vision-to-Results, which you'll read a bit about in this issue. We didn't set out to build a faster methodology. We set out to stop writing documents that decay the moment they're signed, and put our clinical and mission experts where their judgment actually compounds - inside the build, not in front of a requirements deck.
The result, on a recent federal engagement, was a redesign that would normally take a year and a half landing in under three months. I'll be talking more about this in person this fall - at the DoW/VA Health IT Summit in October and at HITLAB Fall Innovators Summit in December - because the conversation happening in rooms like those is the same one driving this shift industry wide: government, payment, and platform all pointing at the same target at the same time.
We built Life365 to be ready for that convergence.

CMS Adds Medicare Options to ACCESS to Help People Manage Substance Use Disorder, Heart Failure, and Other Common Chronic Conditions
Starting spring 2027, CMS will expand its ACCESS Model to cover heart failure, COPD, substance use disorders, and tobacco cessation, plus extend musculoskeletal pain support beyond the initial 12-month period. ACCESS uses an outcomes-based payment approach and is currently available to 160 participating organizations serving Original Medicare beneficiaries with conditions like hypertension, diabetes, chronic pain, and depression; Medicare Advantage enrollees remain ineligible.
AMA Files 171-Page Comment Letter on 2027 Physician Fee Schedule
The American Medical Association submitted extensive comments to CMS on the proposed 2027 Medicare Physician Fee Schedule, pushing back on several provisions including the remote monitoring changes, same-day E/M billing, and maternity care coding. On remote monitoring specifically, the AMA raised concerns alongside its broader recommendations that CMS revise a number of proposed policies before finalizing the rule.
Rural Health Transformation Program: Year 1 Closing, Year 2 Awards Approaching
The Rural Health Transformation Program's first budget period is wrapping up as CMS prepares to announce next year's state allocations. Key dates:
Proposed:
October 2026 – States face their deadline to obligate first-year award funds
September 30, 2027 – Deadline for states to actually spend Year 1 awards
October 31, 2026 – CMS will announce each state's Year 2 (FY2027) workload-based allocation - the baseline half of funding (50% of the pot, split equally among approved states) is already locked in for all five years, but the workload-based half is recalculated and announced annually, about a year ahead of each budget period
Inside Health Policy – RPM makers see drawbacks to current ACCESS model, stress need for provider role
CMS' new ACCESS payment model is drawing concern from digital health leaders who worry it could deepen the split between patients and their physicians, since it relies on consumer wellness devices that often don't connect back to a care team. One RPM executive pointed to companies like Whoop, Oura, and Withings marketing devices as "free with Medicare" while not interacting with the physician. He also warned the model isn't built for complex, multi-condition patients - those with heart failure or other serious comorbidities - because it doesn't provide enough clinical oversight to manage them. The Alliance for Connected Care's Chris Adamec added that ACCESS doesn't offset losses providers face under CMS's proposed RPM reimbursement cuts, and both called for tighter coordination requirements between ACCESS participants and treating physicians.
Becker's Health IT – 35 health systems urge CMS to drop proposed remote patient monitoring ban
More than 230 organizations, including 35 health systems, are urging CMS to drop the 2027 physician fee schedule's proposal to restrict how remote patient monitoring services can be staffed. Organized by the Alliance for Connected Care, the coalition — which includes Ascension, Mass General Brigham, Stanford Health Care, and Yale New Haven Health — argues the change would disrupt care for roughly 1 million Medicare beneficiaries who use remote monitoring to manage chronic conditions at home. The American Hospital Association said the proposal would cause "immediate and significant disruption" to existing RPM programs.
Most delivery methods spend months writing documents before anyone sees working software. V2R skips the document. It turns vision straight into bars - concrete, beatable references - and builds against them with the experts still in the room. On a recent engagement with a global consulting and technology partner, a federal health assessment redesign that would normally take twelve to eighteen months landed in two to three, because the platform underneath it was already wired.
What it does:
Sets bars, not requirements. Every capability is measured against something real - the current instrument, the incumbent completion time, a conformance suite - not a sentence that can be argued down in a status meeting.
Builds and checks separately. AI critics score the work against the bar continuously; a named human expert rules on clinical soundness. The builder never grades its own work.
Lands on a harness, not a demo. Every prototype plugs into the same wired platform - records, identity, devices, accreditation - so it becomes production instead of getting rebuilt from scratch.
Proves value before anyone commits. Simulation runs the capability at population scale against near-real data, producing the value case and a clinical sign-off before hardening begins.
