CMS has finalized the 2026 Physician Fee Schedule, introducing new Remote Patient Monitoring (RPM) codes that expand reimbursement opportunities and enable more flexible engagement with patients.
What’s New – Effective Jan 1, 2026:
99445: 2–15 days of readings
99470: First 10 minutes of clinical monitoring time
These updates lower thresholds for clinical time and reading days - creating new pathways for organizations to be reimbursed for proactive, patient-centered care.
What’s Already in Place:
99453 – Device setup & patient education
99454 – 16+ days of readings
99457 – First 20 minutes of clinical monitoring
99458 – Each additional 20 minutes
